Please read: These forms are for new students who live in the Canton Local School District or for those with approved applications.
Steps to register your child(ren) in Canton Local School District:

Complete online registration. Click the registration button above to create an account for your child (ren).

Registration is not complete until all required documentation has been collected. Note: Parents of students currently attending preschool through Canton Local Schools do not need to re-register for kindergarten. You will receive a separate email with instructions.

As you complete your online forms, you will need to upload the required documentation in the PowerSchool Parent Portal. Please name each file with your student’s first and last name (as it appears on the birth certificate), and include the contents of the file (e.g., Sam Smith Birth Certificate.pdf).

Proof of Residency
- Proof of residency is required for all students registering at Canton Local and must be submitted with the registration form. Parents or guardians must provide two current (less than three months old) approved proofs of residency.
- Primary Acceptable (must include at least one of these documents): a monthly mortgage statement, signed lease or rental agreement, utility bills which include electric, gas, water, trash, sewer, and cable/internet. The bill must show both the service and mailing address and be in the parent or guardian’s name.
- Secondary Acceptable these documents include pay stub, insurance statement, car payment statement, or official letters from government agencies—each showing the parent’s name and current address.
- Not acceptable: Post office boxes, cell phone bills, and bulk mailings are not accepted, as they do not include a service address. Copies of outside of envelopes (see reminder below)
Reminder: When submitting proof of residency, please upload the actual document or letter. Photos of the outside of the envelope cannot be accepted as proof of residency.
Accuracy of Residency Information
- All information and documentation submitted for school registration, including residency information, must be true and accurate.
- Ohio Revised Code Section 2921.13(A) states: “No person shall knowingly make a false statement, or knowingly swear or affirm the truth of a false statement previously made,” when specified circumstances apply. These include statements intended to mislead a public official in performing the official’s duties under ORC 2921.13(A)(3).
- Knowingly providing false residency information to mislead district officials may constitute falsification, a first-degree misdemeanor under ORC 2921.13(F)(1). Upon conviction, penalties may include up to 180 days in jail, a fine of up to $1,000, or both, pursuant to ORC 2929.24 and 2929.28.
