Incoming seventh-grade students need proof of Tdap and Meningitis (A, C, Y, W) immunizations before the start of the school year. Incoming twelfth-grade students need proof of Meningitis (A, C, Y, W) immunization given at 16 or older. Updated immunization records (or an exemption form) must be submitted to the buildings before the new school year.

Any student who is required to take medication during the regular school day must comply with school regulations.
A form must be obtained from the clinic and completed by both the physician and the parent or guardian. (Click here for form)Regulations

Written orders from the physician must include: name of the drug, dosage, time the medication is to be taken, reason for prescribing, probable length of time medication will be needed, and possible side effects

Medication must be brought to school in a container appropriately labeled by the pharmacy or physician

Medication must be stored in the clinic. Students may not administer or carry medicines of any type on their person (lockers, purses, etc.)

A new request form must be submitted each school year and whenever there are changes in medication orders

Medication must be picked up prior to the last day of school. Any medication left in the office will be disposed of if not picked up. School personnel cannot be responsible for medication while school is not in session

Non-Prescription Medication Guidelines
If your child needs to take non-prescription medication during the school day, please follow these guidelines:

Complete the Non-Prescription Medication Administered at School form (link here)

Medication must be delivered to the school by a parent/guardian, be unexpired, and remain in its original container

Notify the school as soon as possible if there is any change in the use of the medicine

Complete a new medicine form if there are dose changes; if the dosage does not match the instructions on the original container, a healthcare provider order is required

If the medication is needed for more than 4 consecutive days, a healthcare provider order is required
Note:
Any unused medication unclaimed by the parent will be destroyed by school personnel on the last day of school—after the parent is notified—or at the end of the school year -
