OHIO HOUSEHOLD INCOME FORM
Canton Local Schools will participate in the Community Eligibility Provision (CEP) under the National School Lunch Program (NSLP). Under this option, all children in the school receive a breakfast/lunch at no charge whether or not they complete this form. However, to determine eligibility for various additional state and federal program benefits that your child’s school may qualify for, please complete, sign and return this application to your school building if your income falls within or below the guidelines listed in the following chart.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:

Summer Meals

What is SUN Bucks (or Summer EBT)?

  • SUN Bucks provides $120 in grocery benefits to low-income families with school-aged children when schools are closed for the summer.

Wildcat Backpacks

  • Wildcat Backpack Form Wildcat Backpack Application – Families must complete a free/reduced application to determine if they qualify for the Backpack program. Once qualified, the backpack permission form needs to be filled out and returned to Nutrition Services.

Food Allergies
